My protocol for injury related fibrosis (and possible venous leakage)

Hi all. Not need to repeat my story which is in the intro section, but in summary, I have just purchased the Sw6 from China today. I'll be starting my protocol the day I get my hands on it. I suffer from ED and perineal pain during intercourse (sometimes) due to a straddle injury which left the proximal third of my corpora fibrotic. Hopefully I can blast some scar tissue away and fix some venous leakage.

I have spent a bit of time on these topics reading through as many protocols as possible and have decided to for with Jchiwoods protocol of 15mj, 10Hz and the 20mm head as it appears he has had the most success. It is promising that others have also seen results with different settings. I would love to know what the optimal setting would be for breaking up fibrotic tissue, but low and slow is a safe starting bet and possibly even the most effective.

I want to focus my initial therapy on the base of my penis where the injury has caused a problem. My theory is that the rest of my tissue is fine so why blast it. I did ask my urologist for a focused session to the fibrotic area but he didn't allow it. 

I know my case is a bit different than most peoples here, but my injury and protocol results may be of interest to others reading who are in the same boat. I'll be keeping you all posted.

Nice to meet you and to read that!

I myself am struggling with fibrotic issues (Peyronie's disease), and just bought an ESWT machine to try to end at least the inflammatory part of this disease (and hopefully, in the long run, break down the fibrotic scar tissue).

I just started tomorrow with low intensity/frequence, and intend to gradually come to medium frequence/high intensity, 2-3 times a week, for 3 weeks, then take 3 weeks off.

Well today was my first treatment day. My SW6 from tplaser on DHGate arrived nice and quick to the UK. It's packaged beautifully, really well built and a piece of cake to use. tplaser were also very easy to communicate with.

I did about 3000 shocks in total and I was pretty much just working up and down the penis, focusing a lot at the proximal third of my penis where my injury was. I was going all the way to the very base at times. Where the penis ends (or begins technically) at the pelvic floor. Perhaps 70% of my shocks were to that area. 5Hz, 15 mj. I have more time to play with, so might reduce the Hz further. I was trying my best to run as perpendicular as possible to the penis at all times, but this got trickier as I got closer to the base.

This was all about 45 minutes ago. I can already feel that light generalised engorgement feeling all over in that area.

I've decided to break away from the consensus a little with regards to frequency of treatments per week. The way I see it, this is a workout for the penis. I work out, and definitely can't hit the gym properly every other day for the same muscles group. The workouts would be subpar, I'd get injured, and not gain in the long run. My sweet spot for the gym is around 3 days for full recovery. If it's restarting a high intensity activity I've  not done for a while, like football, then my legs take a whole week to recover after that first match back.

As this is the first proper shockwave workout, I may give it 5-7 days until my next therapy. Then perhaps every 3 days or so. I'll do a week off perhaps after several weeks and see where I'm at.

I'll keep updating.

Oh and I'm also going to be popping a 5mg cialis on each session day. If things are happening down there, more blood can only be a good thing.

Hey Zapgood!

Love your protocol, love your spirit, love your ideas. I absolutely agree about the workout/recovery cycle, and with the idea of a 5mg Cialis every treatment day, which seems very relevant to me: I'm going to do the same!

Personally for the moment I am at 3 sessions, with no results yet except this indeed nice engorgement that happens for a few hours after the operation. I'm following a high intensity/low frequency protocol though (105 mJ / 2 Hz, 15mm head), so it will be interesting to compare our results after a few weeks man!!

Great to hear from you Gabriel.

Thought I would update as I've had three further sessions since my last. I noticed improvement in erection quality and strength pretty much the same day after the first session. The two main factors I have to go on when assessing erection quality is how big/expanded my glans looks, and for how long I can maintain an erection whilst standing. The glans was certainly fuller, and overall erections better, but not pre injury levels. Standing up the erection went within about 10 seconds. 

My injury is to the proximal third of my penis, at the base, to the left of my penis. I've never since my injury been able to palpate anything discernible other than rough fibrous tissue in that area. Well the next day after the first treatment I was able to palpate an area which felt like a hardened ball that had been squished. Maybe like a pistachio or something just sitting at the base of my penis surrounded by softer (but still coarsish tissue). I've never noticed this before, and figure that the shockwaves have done something the surrounding tissue to allow me to feel it so clearly. This I felt, alongside the improved erections was quite promising.

I commenced my second treatment about 5 days after the first. I used the 20mm head this time on 20Hz, and focused on the injury site again, with particular focus to this hardened lump type area deep in the penis. I'd say around 2000 shots between 3-6 Hz. (I'm experimenting as you can tell).

The lump felt the same size immediately after the second treatment, but by the next day appeared a smidge smaller (I hope). Erections have improved further slightly.

I've just finished my third treatment (3 days after the 2nd). This time I started with the 20mm head at 20Mj, 5hz for about 1000 shots. Then I switched the head to the 35mm and cranked it up to 50MJ for another 1000 shots. Still too early to tell but I'm getting that nice post treatment buzz feeling going on down there.

So a bit more on the technical side of things, I know clinics and some studies run their treatments at 0.09mj/mm2. Ultimately this all early science and nobody knows too much. Maybe the wider head does things differently than the narrower head. Maybe using a narrow and wide in combo at the same treatment stimulates the penis more. Who knows. Beauty of owning our own machine provided it's all safe practice. Likewise with the power regimes. So I"m keeping track of the powers I'm treating with

Session 1:
35mm head, 15mj - 0.017mj/mm2

Session 2
20mm head, 15mj - 0.048mj/mm2

Session 3
As session 2 +
35mm head, 50mj - 0.055mj/mm2

How have you been getting on Gabriel? I note you said that you're doing 105mj on the 15mm head. How are you finding it with that high an intensity coming out of the small head? Any noticeable improvements on your plaque?

Sorry for the late answer, I'm under a sh*tload of work in these times^^!

I've read and re-read your post with great interest. It's good to see that you are experiencing such results already; I'm very glad for you! This is quite auspicious, and I hope things are going to continue to get better with your new protocol.

As far as I'm concerned, I'm not very sure so I don't want to draw any conclusion already, but it seems that the treatment is allieviating some of my pain. As my penis adapted quite quickly to the thing, I cranked it up to 2500 shots (20 min) sessions, every 2 or 3 days depending on how my junk feels (with these years of Peyronie's, I learned to know it and tell when it's physically ready or not for any kind of "exercise", such as sex, masturbation or namely ESWT). So far, it seems that I have, say, maybe 30-40% reduction in pain starting from the next morning of the session and lasting 24 to 48 hours. That said, I want to be very careful, as PD inflammation is very cyclical, so it could as well be a matter of conjoncture.

Apart from that "buzz" and engorgement post-session feeling that you described, nothing to report with regards to erections, but as I stated elsewhere they were not really a problem anymore.

I'll carry on, maybe switching to the narrowest head after a few more session to see if it can intensify the effect, and let you guys updated of course!
